Is it right to ask "What's your good name?"
Unfortunately no. Here are some examples of how you could ask for someone’s name. :)
What is your name? (very common)
What's your name? (very common)
What is your given name? (Sort of common)
(Name given at birth)
What is your god given name? (Not common)
(Name you were baptized as)
