Is "were" or "was" correct?

Which is correct? An Alert and Investigative Summary Report (were) (was) placed in the inmate's record.


I see why you might be confused. It depends on the context. Are there two reports or one report?

For example is there an Alert report and and Investigative report?

If there are two reports then you would use were.

If the report is called an "Alert and Investigative" report and it is therefore only one report then you would use was.
